How to Prepare
Follow these simple steps to make your first visit as smooth as possible.
New Patient Forms
Complete the forms before your consultation. If you cannot complete them online, arrive 15–20 minutes early to fill them out at the clinic. The third-party forms may be in Spanish.
Insurance Information
Bring your health insurance card. We accept most plans and offer CareCredit. We verify your coverage before the visit.
Comfortable Clothing
Wear comfortable clothes for the postural exam and evaluations.
Allow Enough Time
Plan for 30–45 minutes for the full evaluation and health history.
Bring Your Questions
Write down anything you want to ask your doctor to make the most of your visit.
Medical History
Bring previous imaging (X-rays or MRI) and medical reports if you have them. They help us personalize your care.

The Initial Consultation
As soon as you come through our door, we will greet you warmly by name. You will then meet one of our professionals in an exam room. We will discuss your health history, current concerns and wellness goals.
We perform noninvasive neurological, orthopedic and muscular assessments, followed by spinal and chiropractic evaluations and organ reflexology.
Using everything we learn, we begin to design your personalized care plan.
